Please be advised that the online contact lens purchasing service through
has been discontinued, effective April 15, 2015.

Purchases made through prior to April 15, 2015, will be honored with
in-network reimbursement according to your benefits plan. Please continue to use the Reimbursement Form to submit claims for in-network reimbursement
on dates of service prior to April 15.

Beginning April 15, 2015, members will be directed to for online purchases
of contact lenses and other eye care products. Any online purchases of these materials are
considered as out-of-network claims. Please use the Member Reimbursement Form
when submitting claims on purchases made through Vision Direct or any other online provider.

All member forms referenced on this page can be found on your insurance provider's website.

To minimize your out-of-pocket costs for these materials, members may visit the office location
of any participating provider. Please go to the Provider Search section of your insurance provider's
website to find a provider near you.
